    Does anyone know why the "Name" and "Email" fields are missing from the comment form on the following blog post page?

    It's because people have to be logged in to leave a comment. Uncheck the "Users must be registered and logged in to comment" in your admin panel.

    I think I solved the problem. Apparently, it would not log-out because I had multiple windows open. Once I closed all of the other windows containing pages of my blog and then logged-out, it actually logged-out!
