I would never pay to sign up on any first of all. But there are some decent ones, check out my sig link for a list, they will tell you if they paypal. Also check out ebay, its free. Good Luck

Thats called a lead based program, and Commission Junction has a ton of them. They don't pay over paypal though I don't think.
Right well the point is that there are so many affiliate programs out there paying for leads, that really your only question is who pays over paypal. Personally I only have much experience with CJ, but I have seen several programs out there that pay over paypal. I don't use them, but I do know that they are not that hard to find. Infact, try searching these forums for 'paypal' or 'pays over paypal' etc. and you might find what your looking for.
The ones I use below pay and are listed by how they pay, including the ones that pay via PP without a fee!

webcams.com is not pay per free signup . you only get the $25 for every free lifetime member you refer. You are paid as soon as the customer's transaction is approved..
