shopping cart/order form?

I have a beautiful one for on my site I wish to have something similar created.

It might even need to be broken down to a few forms to be able to offer several types of traffic I now have to offer ,maybe even put them on separate pages so I can give descriptions of each type of traffic.

I have good experience with PHP (I can PM examples), but it seems that you use paypal. I've never used paypal to sell products, but it doesn't seem like it would be too hard to figure out.
