    Needed: Price for PHP Reverse Mortgage Calculator & a contact form.
    I have a client who needs a Reverse Mortgage Calculator for a new website. The purpose of this is to collect qualified candidate's emails for further sales.

    Below is a representation of finished product from another website

    Contact form will have verifiable fields for phone & email and will also include name, state, and comment box.

    Anyone have the code for the reverse mortgage calculator? I need to capture the users name address phone number etc, and compute the rm info.
