Well there are several tools to create a website audit report. I find IBP to be good but there are several others to choose from. You could also use a combination of tools and put your finds into one report. That report will effectively qualify to be a website audit report. It should essential consist of the following. Competition analysis, brief ranking report and some keyword research, website analysis in terms of backlinks, PR, search engine saturation etc. You would have to highlight the deficiencies of the website, work needed to be done and the course of action. I am sure if you look for sample website audit report on Google you would find many. Hope it helps. Feel free to ask more.
I've used IBP before and ya there pretty good. it's a good start. Word of caution though don't lay it all out for them many people just want you to tell them what to do, and you'll waste a-lot of time giving away free reports.
Dirty Dog is right. If you tell the client that you use IBP, they will go out a buy it and get rid of you. IBP has a export to Word that I use. I then clean it up into my format and they never know.
