Yeah they must have in some way been invalid. Maybe someone clicked the ad 5 times in rapid succesion and they weren't counted.
I get those clicks too;that's depend what ads format you have, if there is no ads available on your current format it will display a public ad. When you setup a new ad on adsense it will ask you if you want a public ad or a solid color block to show when there is no ad available for your setup. This is why you get click and no earning. You can read more on this topic on google ads setup. This don't mean you account is disable. Here is more INFO.

Proxy ads get very little. I remember for a period i got 0.01 per click. Mine are much higher now though.
These 5 are invalid clicks in eyes of google. You may end with a suspended account if these invalid clicks continue to counted by google.
Well, if each 5 clicks earned him nothing like they did this time, then he would earn nothing. However, I really doubt that would be the case. It depends on which ads were clicked, and how much the advertisers are paying for their ads to be on his site.
