I am looking for a design that follows the below example website outlines. Two things which are really important in my design are: 1) You design space for the form that you find in the header in both the examples. 2) You create into the design something similar to this: EXAMPLES Example #1 - I really like the large header graphic and the navigation about it. However, the form in the header graphic (the one that asks for your zip code) is too small. I also like how they added all the insurance companies beneath the header. Example #2 - This site does a good job of incorporating both the form and the steps into the header design. Though, I think the overall header design is bland. Any stock images you use in your design I am willing to pay for at the end of the contest. Also, the images in the design need to focus around healthy happy people and if you use a logo it needs to be an abstract icon that could be used for anything. All questions and submissions must be submitted through the thread. I will not take any PM-ed designs or questions. This way I don't have to repeat myself and the general contest can benefit from your talent and insight. the 2nd link doesnt work..... what is the theme for your site? could you provide a link to your site? do i just design the form and the header or the whole page (which is including the navbar and body content areas)? and do you have a logo that needs to be incorporated? does the header needs to be animated? do you pay by hour or only for the winner? thanks!!

I would rather have you build something completely independent of the current design. So, no need to taint your your perspective by seeing the current design. @Black Thesis Designs: You don't have to worry about putting anything in the form, you just need design space for it. My programmer when he codes the design will be adding the necessary info of the form. However, the form is going to look most like i5ive.com. The silver navigation doesn't fit with the overall look I would just do sample text (which again the programmer will change out) separated by a pipeline (ie link one | link two | link three |). FEEDBACK @pat3ck: Thank you for your submission. This is a good start. Several suggestions: 1) Use a different image for the header, the current image is too business-like. I am looking more for a happy family look and feel. 2) The three steps blend two much into the header. These steps are the main focal points of the site so they need to stand out. 3) Please design the rest of the page. For the footer could you have it as a sidebar, like what you see in most single column wordpress themes. Again, thank you for your submission. Let me know if you have any questions. ~Paul

Several suggestions: 1) The image needs to go the full lenght of the header or you need to have the form and image designed the full length. 2) There needs to be room for the form on the header. 3) Try to make it so that you can see the image and also the instructions. 4) The image is exactly what I am looking for. See if you can find an image that incorporates happy children, too. @Ace2016: Glad to see that you joined this contest. I REALLY like this design. It begins to capture what I am looking for. Just a few suggestions. 1) I've added below some of the insurance images you need to incorporate into the design. 2) I want to have a footer and a sub footer. The footer is similar to what you created in my last contest, where it was more of a footer-sidebar. The sub footer is what you currently have in your design. 3) We need to also add a sidebar in the body text. I'll let you decide if a left or right side bar looks better. i5ive.com has one on the right. Let me know your thoughts.
FEEDBACK @Macbookman08: Thanks for the changes. The picture looks a lot better. However, there are several problems with this design. 1) The form design doesn't work. To get a good idea of how the form will be programmed look at i5ive.com's form. The 123 steps need to be built around this concept of the form. 2) The navigation doesn't work in it's current location. I would put the nav above the header. 3) Finally, I need to have the insurance companies to be at the top with the header to build trust. It cannot be in the footer. Plus, I need for you to design a space for the form. The best example I can show is the header of i5ive.com. 2) Could you further design the text body and footer.
