I very much prefer a left-aligned design not centered. Other than that; I love the looks of the rest of the site.

 

To avoid getting a lot of spam, I would definitely require the email address and most importantly, I would add a captcha or similar.

And I would add some space between the fields.

As stated earlier you need a captcha for sure and also make email field compulsary ..rest all looks so cool..good work..
