php/mysql script needed

    I need a simple php script where I can add/remove items from a mysql database and display the items on a webpage.
    Instead of explaining what I really need I thought I'd make a mockup:

    The first part is how the results should be displayed, the second part is how the form to submit items to the database should look like and the third part is where I can remove items from the database.

    Notes: Thumbnail images can either be generated when uploading or you can add a new field to the database entry form where I can upload the thumbnails myself.
    I need this script for a friend and won't receive anything for it, so I'm really looking for the cheapest solution here. PM me your quote or any questions you may have.

    I think it's not entirely fair if you try to get him to pick you by already making a draft version, everyone could do that.
