Plz help : How to make Wordpress custom search form

    I need a custom search form for my phone blog. It should have options to ( check box ) check categories which one or which categories to search. How to make it for wordpress ? I will put it in sidebar.

    I actually want search form like Dating sites where you can search profiles checking the box of categories - Age, country, religion etc. I need that form for my readers not for me.

    I am also looking to find a search plugin enables a sidebar widget or a search page for my holiday blog where users can select the name of the location, type of holiday they are looking to buy, number of people etc. by using check boxes, drop down menus etc. refining results. Is there such kind of plugin exists?
