I'm talking about google adwords, so far the most expensive keyword i've found is "refinancing" for which the advertiser has to pay $26.55 to $37.11 per click to be on the first spot! The estimates are based on what people pay for search on these terms at the moment. Sure you can pay less, but these figures show what the highest amount paid is. In other words Google really is getting paid $38 odd dollars for every click they send through these searches. Almost unbelievable but if you imagine what the worth of these keywords are to the advertiser its not so outlandish. For instance "mortgage refinancing" if someone signs a contract for something like this it could earn the advertiser thousands and thousands of pounds/dollars.

That's not quite correct. Yes, they are estimates. But based on average across all advertisers for the keyword. It will depend a lot on your click rate (there's the big estimate), the resulting QS and what it would take at that QS to get in the top three positions. There's no way the top three long-term advertisers have such poor QS they are paying $30. It's likely more like $10-15.
