I think it's by invitation. Your site must have 20 million page views. for big sites via contract with google. It takes from weeks to monthes.

Hi, That's better looking Ads as for as I know which means probably better CTR, I think also better commissions.
They are not open to application. If digital point can't get it you won't be able to I think one advantage is that they remove the "ads by google" text.
They are not accepting anymore premium publishers. You get all parts of the ad clickable, different fonts, and a few more options.

Google will choose you automatically if you fulfill all the criteria. Anyway, it will give you more options like ad size, font, color etc.
there are a lot of benefits of being a premium publisher, like you can have flexible ads units, you can remove "ads by google", fix the ctr as a contract with adsense, etc! the owner of dp has a premium adsense account, other than him i guess shoemoney has a premium adsense account, i have heard that google is not taking anymore premium publishers, not sure though!
