2 accounts are allowed by same name ! Condition is 1 for personal use and other for buisness use No idea about your situation!
you will be banned again if used the same name. no matter if you have different address, the computer and IP will be probably the same and google will find out the corelation. if the ban was for no fault of yours, then appeal and try to get reinstated. if it was for cause, then well deserved and move on
according to Google, you cant. because the moment you get banned for invalid clicks or something, you are not to participate in the program anymore. But of course if you gonna use another address but same name , your application will go through(please use a different website url) but make sure you dont commit the same mistake again. If there is any slightest suspicious that you have cheated them, they will ban you again. This is because there can be 2 different person having the SAME name in the same country, just different location. So there is no way Google can say you are the same person because name isnt unique and you cannot ban a person from adsense just because he has the same name as another frauder.
If you can change everything other than name then I guess it's possible to get another account as it's not that difficult to find people with same name.
