99% of the time you will not get it un-blacklisted and you will most likely just get a generated email response.
When I bought a domain that turned out to be blacklisted: was by auction with no content, so I had no idea. All I did was email AdSense explaining I just bought it and can they unblock it and within a week it was unblocked. If you've broken the TOS yourself though try to remove the TOS violation and email them explaining you've cleaned the site up. That's realistically your only option without more TOS violations like you could for example put the ads in an iFrame on another domain, but this would be a TOS violation (risk your account doing something like that!). First, remove the reasons for the banning and then reapply by sending them a polite and humble request email.
one of my site blacklisted 1 year ago but I republish that and mail google , support , reviewer team delete my site name from black list afte 1month mail progress ! mail and mail and mail them !

this is actually the best way to handle it. Google is not small company. Keep sending them email politely with your previous email below it as reference. Who knows, if they see you persistent enough they may review your site and put it back in the program
