    Good design is convenience. I'm sure I'm not the only one who feels this way.

    I think it's a useful topic, even though some people don't think it's important. There have been many poor submission pages that have made me leave a directory and never return. The main problems are badly written guidelines, too few guidelines or too many, too many fields to fill in, bad captchas, and Adsense or other advertising plastered all over.

    The simpler the better.

    Logical woman.

    As JamieG says, your rules page needs some rules. But other than that your submission page is mostly fine. If you want to make it simpler, get rid of the meta description field because a lot of people who don't submit regularly to directories won't see the point of it.
