I leaned this the hard way. Also, NOT performing muti-variate ad testing and NOT separating your content network and your sponsored search between campaigns. (if you choose to run ads in the content network) Basically, NOT doing anything that is considered best practice could be included in the list.
I had the same problem with not separating Content and search ads. And I always wondered why my quality score was low .

Not hiring a professional........... Would you go to court to defend yourself against a serious charge, or hire someone who knew what they are doing???
Biggest mistakes I've seen: Too broad of terms on a network that is broad Too narrow of terms on a niche venue Not using negative keywords Not taking advantage of advanced matching - Phrase match and Exact match Trying to be number one all the time Not auditing traffic for fraud - This is a biggie. Not using any type of source tracking Not using the ideal landing page to the keywords used. Ads that are too generalized - If you sell electronics, why not focus on categories or even select items? That's just a few I can think of off the top of my head. -Bing
