What am I doing wrong?

First up this is not a cheap shot to try to get hits.

I have a site that has been active for about 7 years, it has always contained correct, current and relevant information always on topic etc.
I have never tried any doorway pages or other spamming tactics to get reusits, just hard work in trying to optimise pages with good data.

I cannot get over a PR3, my keyword ranking is up and down and I am starting to think my site must be fundamentally wrong - any feedback is requested.

    The site is:

First, Pagerank value is always changing behind the scenes. The number you see now is what I like to call the "visable Pagerank", this visual number representing your web page's Pagerank is only updated every 4 to 6 months; this is no clear pattern for visual Pagerank updates. Now, the way to increase your Pagerank is by quality backlinks. A quality backlink would be from a site relevant to yours (for the SEO value) and having a medium Pagerank value with little to no external links beyond yours. A web page with many out bound links will give less Pagerank value to each of those sites.

Moving up and down in search results has been quite common for the past few months for many webmasters. This is most likely due to changes in the search algorithm, which Google does often but very unknowlingly to us webmasters. Changes can effect the current search results position temporarily or permanantly; permanantly meaning when the changes come to full effect in all data centers. Continue to due the two following things is the best way to stay "in the game" after proper on-page optimization, web document structuring and proper internal linking.

    - Provide unique quality content, don't change content on old web pages create new web pages. Changing content on a well preform page should only be done for visitors NOT search engines.

    - Attempt to build quality relevant backlinks; this would be from a site directly related to yours in subject. The content should be unique and the web page should already retain a Pagerank value.

    Backlinks, Backlinks, Backlinks!

Create a context map for your content and category, find "authority" sites to send you backlinks by giving them articles or guest posts.

I think it's your internal linking that's, for a good part, responsible for the problem.

All your internal pages are PR 0/NA ! I suspect your navigation system
ex. (cgi-bin/pagegen.pl?winerylogon) does not pass internal PR.
