Good list. However, point 1 should be discussed further. According to Google, you'll get a better CPC with the 300x250 instead of the 336x280. Like you said, though, many webmasters get a better CTR with the 336. I've played with both, and I'm having better luck with the 300x250. Part of the reason is the better paying ads, but I can also tuck it in a few places where the 336x280 wouldn't fit, thus resulting in a better CTR. If nothing else, it's worth experimenting with.
Great tips, but for the including, you can just include your adsense code with a php include function. That's the way I always do that :0

Even easier is using a php include. Even if your site is built in straight html you can insert the following code into your htaccess file to have it parse php within the html. This way you don't have to worry about if your server supports ssi as almost all support php.
