    (ive not been banned btw)

its just that everytime you hear of someone being banned, its because they broke the TOS, but im sure lots of honest people did it by mistake...i mean, not every person who is new to it can remember them all off by heart.

    do they ever give warnings?

also, if you get banned, are you ever allowed to open an account again?.

Yes they give warnings if they find you violate the tos.

Its only in extreme conditions they ban accounts without warnings.

Not true. You can re-register under a different entity/address.

minor TOS violations will lead to warnings.

any attempt to cheat will get a direct ban.....no warnings for cheaters.

I had a site and one page was reported as copyright violation. So they just stopped serving ads on the site. I deleted all pages on this site and forwarded to another. Still not banned (as far as I know :) )

Sometimes they reminded you, if you broke the Adsense Tos, but most of the time, they will banned you without a warn.
You can't reopen your account, you will get banned again if they caught you.

Register with a different hosting. Transfer the domain to a different register.

Have a copy saved of the WHOIS info for the "old" owner and update it with the new entity WHOIS information. You can send that stuff to Adsense team and they will enable it for that domain, as long as the content is alright during the time they verify it.

    Peace,

Most cases they give you a warning, when you do something that is past a warning you are basically up the creek already.

No body wants lose their adsense account, most of the website owners and i think 95% are very carefull with their adsense account only a few do play tricks, since last 1 and a half year as far as i have seen posts regarding account ban on various forums and google's group, they ban you widout any warning.
before this they use to send email about voilating adsense TOS, like once i get an email about a post of my forum that i have cracks and booter's link on my forum. i didnt remove that post but i remove adsense code from that forum and let it be on my other sites. still i got ban after 1 week. i am still wondering if i removed ads from that forum why they ban my account (very funny)
i did a re apply from a different Computer for a different address, for my other sites which were serving the previous adsense ad, and got another account and it is running ok since last 1 and a half year. :)(mention: not for that forum)
one thing more some time they ban just an account mean you can get another account ad for that domain, and some time they ban everything for that particular website. like you can not put any adsense ad on that domain for ever ( my forum was an example :p )
