How do you setup your clickbank affiiate program

I've been trying to figure this out but it seems a little confusing. After I signup for my "Publisher" account, what do I do next? I filled in the information but my status is still Pending Approval. BTW What do you fill in for the Hoplink? Do I fill in the url to my sales pages? I'm still on Step 2 and 3.

Step 2: Publishers, complete your Hoplink Target URL information on the "My Site" tab.
Step 3: Publishers, complete your Thank You Page URL information on the "My Products" tab.

 

On the "My Site" tab you put your hoplink target - your sales page.
On the "My Products" tab you put your product price and thank you page (the download page).
If your status is pending approval you just have to wait...

How long does it normally take? Also, I thought I had to pay $50 to use their service. They haven't charged me anything so far.
