No need. I have set a gateway for you but most are from US. You can easily find the gateway on the internet if you want to add more carriers.
Is there any cost involved in using the gateway? Any limitations in the numbers of SMS send as well as to which countries?

The default gateway in this script will not charge you. But if you add a new carrier by yourself and use it, well maybe it will charge the recipients
It depends on the carrier your visitors use. Some carriers may limit it and some don't. But, who cares. You can add many carriers. So far, they have never been limited for sending sms from my site.

I can't. You have to find it yourself. I have provided a few in the script. Can't sorry. Look at the website for the default gateway set up in the site. Well, this sms site is good to monetize with adsense since most of your traffic would come from US.
