I assume this is a W8-BEN IRS Form

    Items 1-5 Fill in
    6-8 leave blank
Item 9 - Check box A - Fill in your country - Leave rest blank
    Item 10 - leave blank
    Item 11 - Check
Sign and date - under capacity in which acting (assuming you are an individual) write in "Beneficial Owner"

    - Godspeed

Yes, but Europe is a civilized place and they probably have your VAT number or something like that

for those of us in the wild east, we need to submit this form absolving CJ of any tax liability for our earnings

I don't think anil you need to fill any vat number as it is for only EU countries .whether anybody has a idea how long will it take for CJ to verify and approve my tax form .It's been 10 days since I emailed them the form and I haven't even received a confirmation email ?

This is only for US publishers and the IRS requires every US citizen to report earnings of over $600 per year.
