Here are my December cpm stats, in order of default:

ValueClickMedia (see my sig) - $1,665
cpm (per ad) $0.63 , fill rate 91%

    CasaleMedia - $121
cpm (per ad) $0.47 , fill rate 71%

    CanepMedia - $4
cpm (per ad) $0.36 , fill rate 13%

    CPXinteractive - $9
cpm (per ad) $0.13 , fill rate 100%

I have two ad types on most pages, so it doubles my cpm. And I never use pop-unders, cause they're so annoying. Canep was a new addition in December, & I'm hoping fill rate will improve. The others had low fill % when I first joined, but grew over time.

I used Axill as bottom tier for about a year. They were paying $0.30 cpm with 100% fill when I joined, but later dropped to $0.20, and never counted more than 20% of the real traffic. CPX is doing a much better job of tracking, so I get about 3 or 4x's the $ at only $0.13 cpm.

Many on DP say that Tribal Fusion is good, but they rejected me twice. :(

Some also swear by Adbrite and Bidvertiser, but I could never get more than a few pennies out them. I'd be interested to hear what people are making with the other programs.

I have been trying to get VCM to re-evaluate my site now that I have much more traffic than required but they never return my emails.

Same. They seem pretty superb at ignoring emails.

I used ValueClickMedia a few months ago, but my CPM was around $0.40 with a low fill rate (around 30%). Could you please answer a few questions:
    - What's your site theme ?
    - Are you using inVue or Insterstitial ads ?
    - Are your main traffic from USA ?
    - Are you using their default campaigns ?

    - What's your site theme ? mix of hobbies, collecting, entertainment
    - Are you using inVue or Insterstitial ads ? no, never tried them
    - Are your main traffic from USA ? yes
    - Are you using their default campaigns ?
only on foreign traffic. I delete all the CPC and everything under $0.18 for US, world, CA, and UK as this is preferred traffic. So if they can't fill them with better ads, it goes to my next provider.

My aim is to get valueclick to fill 100% of foreign visitors, and spread the good stuff across my other affiliates.

It's strange. ValueClickMedia has never given me such a high full fill rate. Normally only around 35%. CanepMedia is really crap for me as well, never higher than 10%.

Nope, they all take foreign traffic and even tell you the cpm for each ad/country etc

    US, Canada - $0.01 to $3.50

    European countries $0.04 to $0.72

    Asian/African countries $0.04 to $0.30

I had a similar experience when I began. Couldn't get more than 40% from either one. Very frustrating. I've noticed the last few months that my cpm ads are getting more clicks than my adsense, so maybe click-through-rate factors into which ads are shown.

This past week (first week of January) the fill rates have changed a bit. Valueclick dropped to around 75%, but Casala rose to 83%. So Casala is picking up most of the slack and has been making around $16 per day this week, compared to $4 per day in December.

Out of them listed I would say Valueclickmedia and Casale are the best.

CPX does, and I think Canep does too.

www.Oxado.com takes adult, but it may be only ppc, not cpm

Does your CPM rates depend on your niche too?

Is the above earnings based on monthly earnings or over a longer time period?
Also, do some cpm or ppc allow free hosting blogs like blogger.com as you don't need to worry about bandwidth issues.
