$10 Quick Job - Email Form Coding

    Check out this test page

    I need a coder to code this email form image to look like that. And of course it must be functional and not use any external website / server to send e-mails. I think it needs an independent script.

    I will pay $10 via Paypal when you complete it and I need this done ASAP, so the first one to post here or PM gets the job (of course you must code it well).
