I don't know about that form never used it
I use adsense-support at google dot com
but it could be the same thing ... wait a little then use the email

 

yes, it's ok but one pub id must be used one page

 

actually, check out this link ... adsense guy said it's ok to use multiple publisher id's on a single page...

Yes, it's fine. Google created an API for large sites to take part in revenue sharing.

I've even compiled a growing list of other adsense revenue sharing sites. (see below) \/

 

So true. Everyone has repeated that so many time that it's commonly thought to be that way.

if you know what platform you want it for (php, asp, wordpress, joomla)
I am sure you can search and find it on the net

but the truth is if you know a little bit of programming it should not be that hard to program ...
