There are some good ones in that list and then there are some that don't exist and then there are also some that do still exist, but are very unreliable/non reputable. Thanks for the list anyway.

Most of the companies that you listed are not affiliate networks. You also missed Performics (one of the large affiliate networks) and most of the major CPA networks (Azoogle, COPEAC, Hydra, PrimaryAds). This list is not close to complete in covering the big players, and is unfortunately fairly useless. As you said, some of those companies don't even exist anymore.
