Easy Job: Need Forms Created on Website

    This should be an easy job for a designer. I am looking for someone to create 2 forms for my website similar to this:

    http://www.houstonrealty.us/webuyhouses/

    This will be created on my Joomla site. My site is already created and I have purchased AKO forms in order to make it easier. Let me know if you can do this.
