what am i doing wrong?????help please

im putting a sitemap at my link directory site, and as i upload it in google's webmasters tool i always get an error

This is the error

Incorrect namespace
Your Sitemap or Sitemap index file doesn't properly declare the namespace.


can anyone help me

whats the name of the sitemap ?
ALSO it have to be in the root of the domain
ie
so www.mywebsite.com/sitemap.xml
not www.mywebsite.com/sitemap/sitemap.xml
