it looks simply nice. just if you let the form elements breathe little giving them some spacing will look much nicer.

Nice design and contact form also works fine.
Just 1 suggestion, put that "ajax" div close to the button. So when the button is clicked, the message appears close to it. Would be more noticeable.

I don't think this is done...
It's checking the input in the "message" box only, and that too with javascript alone I think.

I like the form but I think the height of the right hand side of the table would be shorter for better alignment.
