I guess google forced you to change your way of thinking about just adding content.

For my site that was hit with this penalty it seemed to be all about affiliate links. I had a young site with very little reputation and added quite a bit of affiliate code to just a few pages and the site went to page 4 then to page 100 and back and forth in between. Here is the cliff notes version of what happened.

1- added a bunch of affiliate code and went to page 4
    2- removed pages immediately
3- 2 weeks later submitted reinclusion via webmaster tools
4- 1 week after inclusion request returned to normal
5- 3 days later I was knocked back and noticed the pages were still cached
and due to an issue with my site the removed pages did not return 404 error.
6- site remained on page 4 for 3 weeks while I tried everything possible.
7- removed pages and made sure they returned a 404
8- blocked those pages with robots.txt and removed them from cache with google url removal tool.
9- 3 days after properly removing the pages the site returned without a reinclusion request.

I know it could be coincidence but it seemed like google wanted to make sure those pages were gone from site. During this time content was added and links were gained. I know your situation might be different but I hope that you can at least find something to try from my horrible experience.

    good luck

hopefully you can trace back and pinpoint the problem. one thing I learned is to avoid the shotgun approach at the fixing the site. As far as the affiliate links go, I added more than 1 when my site was affected:D Keep a log of what and when you change and be sure to utilize all google's webmaster tools.
