Here's a couple I've used in the past:
Blogsvertise: Requires a 200+ word post with 3 links in it. Allow negative posts. Pays between $5-10 per post after one month of publishing. Offers come every couple of weeks by email, but the more sites you submit the better your odds.
Blogitive: Requires 200+ word post with a given anchor text. Pays $5 per post after a week of publishing. Allows negative posts. Harder to get your site accepted but more offers. You need to log in to check for offers. Requires 2 normal posts between their sponsored posts.

Have you been paid? In what cases are your posts rejected?

    Yes I've been paid.
Blogsvertise are very good. They rarely reject a post as long as you include the 3 links they ask for. My only complaint is that they don't send offers that often - maybe once a fortnight per blog.
Blogitive have more offers but are more fussy. They'll give you a link and anchor text and you have to include those in your post. Doesn't sound too difficult but they often provide anchor text with the wrong spelling or poor grammar and you have to use it - I object to placing typos on my blogs!

V7(user name here is John Scott) Contextual links also pays for links in your blog posts. Blog4money pays for text links in your blogroll.

I've tried all of the above mentioned (although not blog4 money. Can't seem to find that: have you got a link?). PPP provides the most offers although it can be tricky to meet their standards. Blogsvertise is, as you say, very infrequent. Blogitive, their software can be a bit flakey, I've had a couple of posts turned down, not because of any error on my part, but because their own software sent me the offer in error. v7n has only sent me the opening offer for each blog. Not had anything else from them since. Review me hsa never made an offer at all.
As I've got a network of 42 blogs, I was rather hoping for more from the various services.

It's called money4blogs I believe - not blog4money. They'll offer you a price for a permanent (as in you guarantee 12-months) insurance related link - usually in the region $40-50.
