We know key words are relevant but can be different. We may use them as per our need. but same key word should not use more then 2/3 times.

Google said in the last major update that it doesn't take H2 and 3s into account like it did years ago. It still matters but to have it in H1,2 and 3 would be an overkill. As for ranking factors, google sent searchmetrix a chart and H1,2,3 was nearly at the bottom of the list for factors. As for density OP if you go over to the google webmasters youtube channel they out up a video about this... it quotes something along the lines of this. "google does use density as a factor but there is no magic number. It's best to use your keyword in the first 100 words then a couple of other times throughout the article. Make the article as natural as you can'' We need to also remember when calculating density the keyword could be used in other pages on the site and titles etc. Allow about 0.5% extra for this. So if you have a density if 1.5% it's usually around 2%. I use a density if of around 1.2-1.5% and use alot of related keywords instead... Some might say thats way to low but I've has great success with that so I stick to it since it works for me.
These are super old answers since all the recent updates on Google. keep to one or two main keywords, and make your article about relevancy, not about keywords. 1% (6x per 600 words, for example) is best, never go over 2% if you really want your article indexed.

Best practise is, use your SEED keyword once in title, Highest 3 times in body. better use once. Try to keep your keyword at the beginning in your article. If need more then use long tail keyword or LSI version. Don`t over optimized your content. Otherwise google penalized you.
