Okay, that depends upon whether you're using a CMS or a standalone script. Also, your budget. That said, Joomla has a component called JReviews that is incredibly sophisticated. You have to pay a licensing fee of about $100 (at least for the first site - not sure if there is a multisite discount). I've used the trial version, and it is mind-boggling. Now, if you want a standalone script, one that I've seen here is Five Star Review Script I haven't personally installed it, but I have gone to the site and played with it. Very comprehensive as well. The script has a one-time, one-domain license of $125.00. The full pricing options are listed here. If all else fails, visit Hotscripts or SourceForgeand see what's there, of course with the caveat that some of this stuff may not be exactly polished. Hope this helps.

You could try any directory software, directory software is perfect because the directory would link to the site or product you are talking about and also have a listings details page where you can have comment and rating plugins, take a look at phplinkdirectory.com and esyndicat.com and also subrion.com

'design/images/' . $result->rating_image }}" alt=""/> Code (markup): similar to <img src="{{= template_path() . 'design/images/' . $latest_result->rating_image }}" alt=""/> Code (markup): in home_content.tpl I'm not familiar with CI and maybe there's a quick workaround for this...?
