Creating a Demographics Survey

    I've had several potential advertisers ask for the results of a demographic survey... I've never officially done one for my sites, so I figured, since they're asking, time to deliver. Firstly, what is a good program/site I can use to do the survey through? I expect about 500-1000 responses. Can anyone point me to a site that lists what I should probably include in the survey?

    Here are the key points that I would think an advertiser would want to know.

    Sex
    Age
    Education
    Household Income
    Occupation / Employment Status
    Industry employed in

    They would also want to know data that you can collect like monthly unique visitors, their location, your visitors per time of day, repeat visitors, how they got to the website, etc.

    Creating one is easy having users take it is another story lol. Maybe making a contest to go with it.

    surely worth it, taking professional surveys with help of survey companies would cost much more, gifts & giveaways will be affordable.

    I'm not worried about that... there are enough people on my site that will take the survey... I just need an easy (not-expensive) means of doing it.
