Top 5 page for this search query is full with spam results, where most sites are redirected to a group of specific sites. I did submit spam report before to Google, but all results are still appearing as it is. Many genuine sites won't able to rank top due to these results. Let me know what we can do here? it doesn't matter if they are hacked or not, Google should not display top results for viagra on domains like "thehindutemple.org" or "au.edu"
Because they are not hacked yet. I have got list of sites which are redirected if you click to them from Google / Yahoo / Bing. But if you open them directly then they will open normally. Hackers redirected them only if you click them from major search engines. If you view coding source of Google cache page of that given hacked page then you can also see spam there. But if you can directly analyze source of a site then there is no spamming. That is the work of SEO that's why Google failed to give the best result because of some SEO using unrelated keywords in their site that might give good results in traffic.
Google is well aware of all of these sites. They fix what they want to fix. There is a lesson here but most people choose to ignore it. If you want to be at the top for niches such as this, you are seeing what it takes. Turning people in and thinking you're going to get there playing 'by the rules' is naive.
