try to be authoritative, try to state the value of your website, if they think you website has value and has authority they will never ban you, instead they will try to work with you to solve any upcoming problem

You can try make high quality content, unique. Follow their rules and put adsense policy on your site. Monitor your backlink, do not let junk site or dangerous website linked to your website.

Having a new account might be a good idea for what you've done before might still be on the record.. Then after that, just follow the guidelines and it will run as long as your on the right track

just make sure you follow the rules, don't click on your own ads, and don't try to spam it with other IP
try not to get too much backlinks at a time, because your website might not be able to handle it, and this might get Google to pay more attention to your site

Do the opposite of whatever you were doing before, or just follow NetStar's excellent advice.

I think it's pretty much common sense. High quality content, on-page SEO and avoid cutting corners in general. If Google doesn't punish you for something now, doesnt mean they won't later.
