Well first thing is to keep in mind is that Quality Score is assigned to Keywords only not to the Ads. Quality Score of your keywords is the most important part of your campaign optimization. Google stores the keywords performance for first few days from the beginning of your campaign, then it will increase or decrease depending your keywords performance. To improve your keyword's QS you need to keep 2 things in mind. 1. CTR of the keywords. 2. Relevancy of your keywords with the Ads & Landing Page.

Yes, but apart from that; there are various other factors listed above which really affect the quality score of the keyword.
Not only CTR, but also relative CTR, for example if you are in position 5 or 6 and you are receiving a respactable CTR relative to others in the same position this will benefit QS.
The first things that comes to my mind are ad copy and ad-keyword relation, but there are many other factors: Search terms - in some cases it can be helpful to exclude certain search terms which have low CTR. Geographic location - in certain areas your CTR might be lower than in others Device - desktop, tablet or mobile Network - note that search partners' CTR doesn't affect Quality Score
Please bare in mind there is 7 different types of quality score for example: acount level, adgroup, keyword, ad-level, landing page, display network and mobile.
