I have used my website name to search on Google preview tool. If I search from USA IP, my website is rank #2, but in UK or other IP, my site is Rank #1. And I have searched by combo keyword, I can see my ranking is 5 pages away in US... But UK, Canada and Australia results are in first page... Does google penalty my site only in USA? How can I deal with this? Thank you very much
It could be the new update. It's only been rolled out in the USA so far and lots of sites have lost a significant amount of SERPs.
Keep patience and try to find out what you have done for search engine ranking where there is any kind of thing which may make your rank down. I think it will be all right just see what google is doing with respect of time.

is it a country TLD or is it a first level TLD? where is you majority of backlinks come from? Some people believes that UK search is a preview of whats to come in the US

There was a google update last Feb. 7, 2011. Your site could have been affected. They might be prioritizing the Google US search engine. Just continue to build Quality Backlinks and you don't need to worry about it too much.
