Depends on what the cpm is, if the cpm is $1, then your example is pretty much spot on. Some high traffic sites get cpm as high as $10
Depends on what you're monitizing with, billion page views on Amazon is worth more than on Facebook because of the nature of the business.
If you want a definite answer, I'd go for false just because it's such an open ended question - it totally depends on what site you're running. A million hits to a sex or gambling site is going to pay off a lot more than a million hits to a Justin Bieber fan site.

Of course it can, if that's your standard rate for private advertising sales. There's a lot of companies that'd be more than happy to spend $10 CPM for quality page views. Lot of companies that take up large media buys aren't interested in profiting directly from the click throughts, rarther than just getting exposure for their brand name.
