That is Dosh Dosh's ProSense theme which is available at http://www.doshdosh.com/wordpress-themes/Prosense-Adsense-Ready-SEO-Theme

It is old and overused quite frankly. Has even been adapted into a Blogger theme.

You can recreate it fairly easy. It is pretty basic.
