I suggest you to put your main keyword in title of your Home page like soccer jersey and for inner pages do like this soccer jersey england national team, same for other pages too. Thanks !

Before doing anything I would advise that you really understand the keywords and phrases you are interested in targeting. How many exact searches/mth, how much competition for them, how can you out rank them? That's the most important step, then you can add them to your pages and titles as mentioned.

You need to have another keyword for every page that is related to the product but always try to include your major keyword in every pages of your site.

You need at least one or more keywords in every page of your website, for SEO purposes. If you do that (especially with low competition), you will get results and traffic base on your performance.

Not necessarily. Your keywords should be spread across your web pages or website so to say. Product reviews, articles, etc. could have different keywords spread in an optimum way. Optimum way is something that you need to decide. But it is important that the keyword is used at least once for every product and more times for product description. Judge that as a reader than a marketer.
