Takeaway form with checkboxes

Hi All,

I only know little about PHP and urgently need a fairly simple form for a takeaway restaurant thing.

So would just need to be able to have check boxes on a list of items, and basic info from client such as name address etc...

Does anyone know where I could find such form for free?
