    The first screenshot shows unpaid balance accumulated in July 2010. You can withdraw via check from Amazon, once your unpaid balance crosses $100 or more.

    Second shows your earnings of $7.30 from 3 items shipped by Amazon. 'Month to Date' means the current month. That is from 1st September till today. Remember, you only get paid when items get shipped.

    Third shows earnings of last 4 months. That is, the period from the first day of the current quarter through today. Each year is divided into four quarters: Q1 (January 1 to March 31), Q2 (April 1 to June 30), Q3 (July 1 to September 30), and Q4 (October 1 to December 31).

    Yes, you can receive gift card ($10.00 minimum earnings). Just set your Payment Preferences to Amazon.com gift certificate/card.

    You can hold payments just like adsense in that same screen.

    Yes, just set any higher value to whatever you want. You'll not receive checks until it reaches that amount.

    Gift cards are sent to your primary email address, at the end of the every month, provided you've reached $10 minimum earnings.

    I think Amazon accepts HDFC netsafe cards. But, better contact Amazon before purchasing.

    @OP.
    Good luck on this achievement.
    Can you tell which niche you are in, or which site of yours you made this revenue from?
