this shouldn't be in adsense. anyway, most CPM networks require approval. I'm shocked that any allow you to get in without needing to get approved. if you want something with more or less instant approval, move away from CPM & CPC: try affiliate marketing.
Similar to Adversal is paypopup which pays similar money and is aproving most sites. Those that denied your site usualy need high traffic site with specific niche or country of origin.
CPM networks have to take on a much higher level of risk with each sign-up compared with CPA. They all require approval for that reason, generally the lower the rates they pay the easier it will be to get approved.
It would be extremely rare for any CPM to just approve any site, as they are paying per volume regardless of what the visitors do, leaving room for them to be easily ripped off
