I have reviewed couple of campaigns I have done in the past. None of them are 10/10, they are mostly 7/10 and 5/10. What would you recommend doing?
Read Adwords FAQ with all the details. QS is heavily influenced by your CTR. Your CTR depends on how attractive your ad is to the reader. Increase CTR, increase QS. A 7 is not all that bad, I'd say slightly above average and 5 below average. The goal of course is to get higher since it reduces your costs.
In my opinion make sure you have muliple ads setup to match the content the users are landing on rather than a broad ad covering lots of keywords. By simply doing this i went from 7/10 to 10/10
Quality score is usually dependant upon several things. Make sure your landing page is relevant to your ad and use only a few tightly focused keywords in each ad. Test different versions of your landing page to see if one works better than the other. Also test different versions of you ads. You want to increase your click through rate and this will increase your quality score. By continuously testing your ads you will see which ones get clicked on the most.
The easiest way to increase CTR's is changing a/b testing ad text. Make sure you are using keyword insertion, ie {KeyWord:default}
Thats taken care of. Thanks Any hints on destination URLs? I've heard that adding ?keyword at the end of destination URL increases QS.
Not sure about destination URL's but display URL's can have an impact, again it is about making your advert more attractive, increase CTR and therefore increase QS once again.
You might also temporarily pause or delete any keywords with a QS of less than 7 to raise the average click rate for your account. Set a target for the average account CTR and keep raising the target. You should start to see some 10s within a month or two if not sooner.

QS is determined by many factors, including account history, campaign history, CTR, landing pages and a few more. At the beginning, the landing page matters most, make sure you have unique content and no cookie cutter affiliate page. 7/10 is pretty much the average you'll get you you start, 8/10-10/10 are rarely seen from scratch, but with a good CTR you'll head in this direction. Start slowly, not too many Keywords per adgroup, if you pump an account with 50000 keywords at once and think you are smart, you'll fail.
