Need a Custom Email Opt-in Box Made

    I need a custom made email opt-in box script made for my WordPress Blog. It doesn't need to be anything super fancy, it just needs to look neat with the caption saying "Get my 5 day Online Training Course sent directly to your email by entering your name and email address here" and have an arrow pointing to the box.

    I am willing to pay $15 for someone to make this for me. Please PM me and I will give you more details about the project.
