    I'm one of those persons that look at the efficiency first and at the design second. So, in my opinion, if you hit your objective, the website's design is good enough.

    I would work on a new header that calls attention to the site and I would also advise not using a domain that has a trademarked name such as PayPal, paypal will not appreciate this and they will make this an issue once they find your domain.

    I would also advise getting proper validation.

    I think the page works well as a lead capture page. If you like how your site is converting then I wouldn't change a thing.

    However if your conversion rate is low then a few things I might change to help boost conversions are...

    1. The design of the site. The design is basic and might convert better if it looks more professional.

    2. The payment image. Looks a little distorted and people might think its faked. Make it clearer.
