Dont mention too much the keywords that you are targeting. For example, if you are targeting the keyword "web design" just mention it in your article at least 3-4 times. 3%-4% Keyword density is good enough
I suggest you please don't use keyword stuffing in articles. you should write article for back links that every article give you two one way back links. don't use hug keywords. you should increase keyword density in your website's content that will help you to ranking.
That depends on the host where you submitted it, not us. We have no clue where you submitted your article. Maybe you should read their guidelines?
If you know the problem then minimize the keyword density, such types of articles are not approved by most of the directories. Before submitting article you have to read editorial guideline and followed them. This is also a major cause of article being not approved. Keyword density is approx 3% is good and varies according to directory guideline.
Ezine Articles is like that, but they don't tell you what their ambiguous keyword density limit is. I think keyword-density writing holds no value anymore, personally... much better results writing naturally. Instead repeating keywords a buch of times, try using a tool like http://www.quintura.com and use some keywords that show up in the copy of top-ranking pages related to your primary keyword. I've been doing it for 3 years, and it works wonderfully for me.
Unwanted or intensional repetition of keywords may also results in loosing the reader's interest. Try to use pronouns to reduce the kw density. Roughly 2.5% - 4.5 & density for major kws works better in general.
many of the articles directories dont let the articles submitted if the keypwords is repeated more than 5,6 times..like ezinearticles.com
There will some online tools to check the density. Check it Google or you can use tools.seobook.com/general/keyword-density/
