Has your site ever been removed from Google indexing? And did you get back in?

I was. And got back in by contacting Google and explaining the situation. I had an overly aggressive SEO guy mess things up for me and my site was banned. This was around 2001. That was a very black day getting kicked out since most of my business comes from Google.

    What you got?

I deleted everything from the homepage, waited for it to be indexed, then put some new text on, waited for it to be indexed again, asked for an inclusion and got in!

As i know Google seldom recover the index once the site was put into the rubbish bin,
what i have done it's remove the whole site to another domain, the the result is PR and index back both,
anyway i don't know why your site would be deindexed,
it's would not work for duplicate content issue IMO. Good Luck! HAHHA! :)

Try to find the exact reason of bann, better if you change the look of your website according to the guidelines of google. The request for reinclusion. The process takes time.

If you got deindexed and were back in a week i highly doubt that was a ban, penalty or de-index, more like a reshuffle that you paniced on when you say you weren't on page 1 for your term anymore.

Most de-indexing takes months, years if that.

It takes a lot of time to be indexed and has a good ranking.Just do social networking and some SEO tool and it generates a good traffic.

My inner links were on the front page. But after the last update, I lost a lot of rankings. Hopefully, they'll come back in a few weeks :)

    I faced that once.
Removed the sitemape and deleted the indexed paged from Webmaster Tools.
Then, after a few change and update, I have submitted the sitemap again and it came back.

The SERP took about 2 months to get back.

I have had sites removed from the index a few times, some made it back, some have not.

I think everyone would have to be removed from index at some point on one of their sites, these are just the lessons one must learn as they become better at SEO.

My first and most major mistake was using a product called 'traffic hurricane' -- basically what it did was create tens of thousands of spam pages using re-hashed, recycled scraped content, using basic on-page seo to target long tail keywords.

While this technically did work, and did make about around $45,000 USD before my domain was banned by google, making me $300-500 a day on google adsense (I did the entire jewelry niche, targeting pretty much every jewelry keyword)

Well anyway, like anyone who uses that program, I was de-indexed. The main site itself was getting 5000 on average unique visitors per day and was an authority site in the handbag field.

I tried countless times asking to be re-approved explaining the mistake I made messing around with the traffic hurricane script. Google never let me back in and I no longer own that domain.

One time I purchased a very high traffic domain (baseballgloves.com)

Well it turned out the previous owner got the domain banned from google because 8 weeks went by and the never indexed my site.

It took me a good 6 months of leaving that site blank and requesting a reinclusion.

I also have been penalized a few times on some sites, where I would rank much lower than I should, like being on the first page for allinanchor: "keyword" but being number 990 literally on the last page in the serps.

No clue on why that was sometimes, I assumed at the time some flaw in my linking strategy.
