This is a long story when i got banned 4 years back from Google adsense. But this time, I have started a new site from my wife's name and have applied for adsense with my wife's name.

Address and Phone no. all are different but still haven't got any response from adsense.

You should most probably get account.

make sure the site you are applying adsense account for is not having duplicate content or anything which is not recommended by google in order to make website good.

    Dear mit,

When adsense rejects your application, they don't send out email to inform you that. So, You need to check if your application has been rejected or still not reviewed.

To do that, simply go to https://www.google.com/adsense/ and try to log in using the account details you used to submit the application.

If it said "This account has been disabled" Then that means your application has been rejected [You'll need to check what's wrong with your site].
    If not then keep waiting.

Use a different home address. Also the address of the website owner should have the address that matches the address you indicated.

Oops! I guess that's a sign of being rejected.

When it takes you to the previous page again, log in (again) from the same page you got redirected to.

Same IP? If so then they will catch you. Changing emails and names are easy.

My suggestion is to wait for another week, and if still no reply from them, try to re-apply.
